Actually it would be more of a Miko Mode. Atleast that is what it is called in her profile. Interestingly enough, this form is just called Sakuyamon on the card where it appears.

It is only called a Miko Mode in the profile. She does not use the name otherwise. And it is NOT a Kabuki Mode. Calling a Miko a Kabuki Dancer would be similar to calling a Nun a Flamenco Dancer. That name is completely fan-coined and wrong.
